McLean's multidisciplinary programs treat a broad range of psychiatric illnesses across the full continuum of care.
Under the direction of the Program Director, provides support to the treatment teams at the program level of care in the area of after-care planning and resource information.
RESPONSIBILITIES AND STANDARDS OF PERFORMANCE
* Assists in various aspects of after-care planning.
* Develops and maintains a database of resources essential for discharge planning.
* Under the direction of the Program Director, provides information directly to patients and family members where appropriate.
* Makes follow-up telephone calls regarding discharged patients who fall into the high-risk screening.
* Functions as an effective member of an interdisciplinary team.
* Demonstrates behavior, which exemplifies the NASW code of ethics to ensure quality of care and promote positive program and department relations.
* Contributes to the efficient operation of the program.
* Contributes to the philosophy of quality care and human dignity of the hospital.
* Maintains confidentiality of information of the patients.
* Provides information to the public, other treatment center/hospitals about McLean treatment and services, often times provides referrals to the facilities with site visits.
WORKING CONDITIONS
Working in a residential setting, interacting with patients who may have expectations for aftercare placement that is not realistically possible. Supervision provided.
PHYSICAL AND MENTAL EFFORT
A moderate amount of walking and climbing stairs may be required. The multiple demands of working with a difficult patient population and a multidisciplinary staff may lead to a moderate amount of stress.

Qualifications
High School Graduate required. Preference given to BA or BS Degree. Basic interviewing and interpersonal skills, computer literacy helpful. Equivalent of one year of experiences in a substance abuse setting, knowledge of treatment resources a plus. All employees must possess basic computer skills to use a variety of electronic or online systems for communication, clinical and administrative purposes.
